Summary and findings

A 72-year-old man with advanced prostate carcinoma was diagnosed via supraclavicular lymph node biopsy despite negative prostate imaging and biopsy results. Rising PSA levels were noted, but conventional diagnostic methods failed to detect malignancy. The case underscores the importance of comprehensive diagnostic approaches in prostate cancer.

Abstract

The authors’ words, as Urology case reports supplied them

A 72-year-old man, who was under regular urological care, was diagnosed with advanced prostate carcinoma, based on a biopsy of a supraclavicular lymph node. Despite rising PSA levels, the patient had negative results on digital rectal examination, negative repeat prostate biopsy, and prostate MRI showed no signs suggestive of malignancy. A PSMA PET/CT scan, which was not indicated for the patient during the diagnostic proces, would have facilitated the diagnostic workup, particularly in detecting metastases. This case highlights the need for a broad diagnostic approach and the use of all available diagnostic methods in the diagnosis of malignant diseases.

Background

Prostate cancer diagnosis often relies on PSA levels, digital rectal examination, and imaging studies. However, these methods can sometimes fail to detect malignancy, as seen in this case. The study highlights the potential role of comprehensive diagnostic approaches, including advanced imaging techniques, in identifying metastatic prostate cancer.

Methods

This is a case report of a 72-year-old male patient with advanced prostate carcinoma. Diagnosis was made via biopsy of a supraclavicular lymph node. Conventional diagnostic methods, including digital rectal examination, repeat prostate biopsy, and prostate MRI, were negative. Rising PSA levels were noted, but no PSMA PET/CT scan was conducted.

Results

The primary observation was the diagnosis of advanced prostate carcinoma through a supraclavicular lymph node biopsy. Despite rising PSA levels, conventional diagnostic methods failed to detect malignancy. The absence of PSMA PET/CT scan limited the diagnostic workup.

Interpretation

This case suggests that conventional diagnostic methods may not always be sufficient in detecting metastatic prostate cancer. The findings align with literature advocating for comprehensive diagnostic approaches, including advanced imaging techniques. However, the lack of PSMA PET/CT scan and the single-case nature limit the broader applicability of the findings.

Key findings

  • 72-year-old man with advanced prostate carcinoma.
  • Negative digital rectal examination and prostate MRI.
  • Negative repeat prostate biopsy.
  • Rising PSA levels despite negative imaging and biopsy.

Limitations

  • Single case report
  • No PSMA PET/CT scan conducted
  • Limited generalizability
  • Potential diagnostic bias
